Overview

 
Soaring Eagles Home Care is a Personal Care Option Home Care Agency serving the needs of the elderly and persons with disabilities throughout New Mexico, and through the AHCCCS Program in Arizona. Soaring Eagles Home Care provides a range of Personal Care Attendant services which enables our clients to promote their independence at home. Our dedicated staff will do their best to ensure the quality and continuity of care our clients deserve.

Soaring Eagles Home Care Philosophy 

Soaring Eagles Home Care believes that health care is a basic human right. It must be available, coordinated, and provided in a comprehensive way, combined with human services when appropriate. We believe home health care is an important part of the continuous health care system, and it shall be provided in the most cost-effective way possible. We believe the services should be tailored to meet the needs and wishes of the clients we serve.

 

Soaring Eagles Home Care and our staff operate and provide services in compliance with all applicable federal, state and local laws. To the best of our knowledge, based on our Quality Improvement Program and professional personnel practices, our services comply with professional standards and principles.

 Soaring Eagles Home Care Goals

 
1)  To provide all levels of home care service in collaboration with physicians and other members of the health care team, allowing clients to remain in their own homes.

2)  To provide a broad base of services based on our clients’
needs, involving clients in establishing, implementing and evaluating services.

3)  To maintain clients in their homes as long as possible by
providing home health care and related services.

4)  To assist clients in using all restorative methods, tools
and procedures to return to their normal pattern of living as soon as possible.

5)  To provide cost-competitive, quality services.

6)  To develop a relationship with referral sources to effectively meet the home care needs of clients.

7)  To serve as a link between clients and available community resources.   

 
Soaring Eagles Home Care Purpose

We believe that home health care services are an essential part of the health care delivery system. By providing continuing care to our clients in their homes, home care services minimize the effects of illness, accident or disability.
 

In providing care to our clients, the Agency recognizes that clients and their families have certain rights to self-determination. They have the right to participate in planning for their care, to make decisions about their health care, and to maintain their independence as long as possible.


Health education is an integral part of Soaring Eagles Home Care’s home care program.
 

Soaring Eagles Home Care recognizes that the behavior, attitudes, knowledge and understanding of our staff have an important influence on helping our clients attain and maintain their health.
 

Soaring Eagles Home Care recognizes the worth of all persons, including the elderly and those with disabilities. We believe the contributions of these persons are no less important than the contributions of other members of our society. We are dedicated to helping these persons retain or regain their maximum participation in society.
 

We recognize that the aging process is an unavoidable part of life. We understand chronic disease and disability may be a part of that process.
 

We are committed to the rehabilitation process of the aged, disabled, and/or diseased individuals within their home environment, allowing maximum contribution, retention of self-respect, and independence.
 

When rehabilitation or recovery is not realistic, we commit to providing dying individuals with appropriate care and coordinated services, supporting their dignity and self-respect.


Soaring Eagles Home Care Objectives

To provide:

·        A coordinated team approach to therapeutic and rehabilative health care services using:

                   - Personal Care Attendants

                   - Personal Care Attendant supervision

 ·        Home health care services and training that allow clients and their caregivers to assume personal responsibility for the client’s health and personal needs.

 ·        A review vehicle for continuous examination of the care a client is receiving from Soaring Eagles Home Care.

 ·        Educational opportunities within the health care system for employees through an ongoing in-service program, short term training for students, and participation in professional organizations.

 ·        Coordinated liaison with health or human services agencies to meet the needs of the community through joint efforts, meetings and community awareness programs.

 ·        A physical work environment favorable to maximum employee performance.
